Iowa State Penitentiary

Contractors and service providers to Iowa State Penitentiary are required to have varying degrees of training and orientation depending on the level of offender contact you will have while onsite. Contractor is defined as an outside or third party who provides a contracted, repair or service to the institution on a limited basis.

All contractors and service providers are required to be have a background check ran before admittance to the institution. You can find the form below, fill it out in its entirety and follow submission instructions located on the form.

In addition, depending on the level of contact you may be required to attend an in-house orientation such as with volunteers. More information can be found here. If so, our staff will contact you to schedule the session.  If you are unsure what level of orientation you are subject to, your institutional contact will be able to tell you.  If contact is minimal you may just be required to read and acknowledge the documents below.

Authorization for Release of Information
Every attempt should be made to return this form to the institution 72 hours prior to arrival.
